Cover and administrative formalities

Depending on your situation, you will need to bring :
- If you are covered by social security: your valid social security card.
- If you are entitled to social assistance under the CMU (Caisse de Maladie Universelle) or if you wish to receive it: the decision to admit you to social assistance or the decision to admit you on an emergency basis, which can only be issued by the social assistance office of the town hall in your place of residence. In all cases, you must sign the document requesting admission to care. An administrative investigation will then be carried out into your resources and those of your ascendants and descendants.
- If you are in receipt of home medical assistance: a form signed at the social welfare office of the town hall where you live.
- If you have no fixed address: your carnet de circulation.
- If you are a pensioner under article L115 of the Code des anciens com-battants et victimes de guerre: your free care booklet.
- If you are a national of another EEC country: your European Health Insurance Card (EHIC).
- If you are a citizen of a non-EEC country: you will be sent an invoice at the end of your stay.
- If you are affiliated to a mutual insurance company: your valid membership card.
If you are unable to come by your own means, the doctor will prescribe the most suitable means of transport. You will then be able to choose your carrier.

General information
- A cloakroom with a personal locker is available on the ward (see staff for details). Items that are not essential for the duration of the sessions should be left there.
- Weighing is the responsibility of the nursing staff who set up the generator. You should not take offence at what may seem like a surveillance system imposed by the doctors.
- Unless special dispensation is granted, visitors are not permitted during sessions: please inform your family and friends.
- Smoking is not permitted inside the department.
- WiFi access is available free of charge.
- Snacks are served during the session. Please let us know if you have any religious, dietary or allergy concerns.
